Building Presibo: The Technology Behind Continuous Healthcare

Healthcare is still largely reactive.
A patient checks their blood pressure when they feel unwell. They visit a hospital when symptoms become serious. A doctor sees a snapshot of their health during an appointment, makes a decision, and the patient goes home.
But chronic diseases like hypertension and diabetes do not work on appointment schedules.
They happen every day.
That is the problem we are trying to solve at Presibo.
From a Healthcare App to a Connected Care Infrastructure
Presibo started as a software idea during COVID-19, initially focused on prescriptions and helping patients stay on top of their medications and appointments.
Over time, we realised that the bigger problem was not simply access to prescriptions or consultations.
It was continuity of care.
How do we know what is happening to a patient's health between two hospital visits?
That question led us toward building a technology infrastructure that connects the patient, medical devices, AI and healthcare professionals into one continuous loop.
We call this the Presibo Care Loop.
The Care Loop
At a high level, the architecture looks like this:
Patient → Device → Data → AI → Risk Detection → Triage → Healthcare Professional → Intervention → Patient
The loop then continues.
Instead of waiting for a patient to arrive at a hospital with a problem, the system is designed to continuously collect health signals and surface situations that may require attention.
Connected Medical Devices
One of the biggest steps in our evolution has been moving beyond software.
We are developing connected blood pressure monitors and glucometers that can communicate with the Presibo platform.
A reading taken from a device can be transmitted into the patient's digital health record, allowing the platform to build a longitudinal picture of the patient's health rather than relying on isolated measurements.
This changes the question from:
«"What is the patient's blood pressure today?"»
to:
«"What has been happening to this patient's blood pressure over time?"»
That distinction is extremely important in chronic disease management.
The AI Layer
Raw health data by itself is not particularly useful.
The challenge is turning thousands of individual readings into meaningful information.
Our AI layer is designed to analyse patient data, identify patterns and flag readings or trends that may require attention.
For example, instead of simply storing:

the system can look at the patient's history, previous readings and other available signals to determine whether the reading represents an unusual change that should be escalated.
The goal is not to replace doctors.
The goal is to help healthcare professionals see the right information at the right time.
Intelligent Triage
Once a concerning signal is identified, it can enter our triage and escalation workflow.
The severity of an event determines the appropriate response.
A relatively low-risk situation may result in a notification or recommendation.
A more concerning situation may be routed to a nurse or health attendant.
Higher-risk situations can be escalated to doctors or specialists.
Potential emergencies require an entirely different response pathway.
This creates a technology-driven hierarchy of care rather than sending every patient through the same workflow.
The Human Layer
One principle is extremely important to us:
AI should assist healthcare professionals, not remove them from the loop.
A doctor should be able to see the patient's relevant information, understand why an alert was generated and take the appropriate clinical action.
Presibo therefore connects technology to people.
The system is not simply:
Device → AI
It is:
Device → AI → Triage → Human Response → Care
That human response is what turns monitoring into healthcare.
Building for Africa
Building health technology in Africa comes with a unique set of constraints.
Internet connectivity is inconsistent.
Healthcare infrastructure is fragmented.
Medical records are often disconnected.
Patients may move between pharmacies, clinics and hospitals.
And affordability matters.
This is why we are building Presibo as both a digital and physical healthcare infrastructure.
The click is the software, AI, connected devices and digital healthcare experience.
The brick is the pharmacy, hospital, clinic, community health worker or physical healthcare professional.
Technology has to work within the environment people actually live in.
Where We Are Going
Our long-term vision is bigger than a blood pressure monitor or a health app.
We want to build an infrastructure where healthcare becomes increasingly continuous.
A patient should not have to wait until something feels wrong before their health becomes visible to the healthcare system.
Devices collect the signals.
AI helps interpret them.
Triage determines priority.
Healthcare professionals respond.
The patient's history stays in the cloud.
And the loop continues.
That is what we are building at Presibo.
A continuous care infrastructure powered by connected devices, AI and human healthcare professionals.
We are building it from Nigeria, for Africa and eventually for anyone who needs continuous chronic disease care.
And this is just the beginning.
